Illness Screen Report

As of February 3, 2025, the non-emergency COVID-19 prevention Regulations (8 CCR 3205) have expired, with the exception of recordkeeping requirements under Section 3205(j).

All employees who test positive for COVID-19 can use the Illness Screen Report to report their cases.

This survey should be used by employees in the following conditions:

  1. Employees who test positive for COVID-19 should use this survey to report their results.
  2. To report COVID-19 Symptoms – employees who have COVID-19 symptoms that are not due to another known or chronic condition are advised to stay home and monitor their symptoms. If symptoms last for more than a day or two, it is recommended to get tested.
